FINANCIAL INFORMATION

We accept most major medical insurance policies with the exception of most  Humana plans.  

 In the event you are without insurance we would be happy to provide you with an estimated self pay pricing.  All procedures must be paid one week prior to surgery.

 Every patient will be required to pay his/her estimated financial liability on or before the day of the procedure. The out of pocket financial liability is different than that of what would be paid to your Physician's office. We will verify your insurance benefits prior to the procedure day.   All quoted fees due prior to your procedure are an estimate of the anticipated service. The actual costs will be based on the services rendered.  Please consult with our center’s business office for further information. 

 We have billing staff available to provide you with a personalized estimate for your procedure, upon your request. This request will include but is not limited to any insurance deductibles or copays, as well as, Anesthesia or Facility fees. Certain procedure may be accompanied by out of pocket products. These prices will also be included should they apply.  

As a courtesy to all our patients who chose our center for their procedure, we will file an insurance claim on behalf of the patient's insurance plan.  A patient is expected to pay their financial obligation prior to the surgery procedure. Should the insurance company sent an explanation of benefits that requires additional payments outside of what was collected prior to the procedure, the patient is required and expected to pay that balance in a timely manner. Any remaining portion not paid after 60 days will be referred to our collection agency.  Any balances sent to our collection agency will have additional fees added in accordance to the collection agency's policies. If a patient is referred to a collection agency, they will not be permitted to have future surgery procedures within our facility.

Services may be provided in this surgery center by our facility and providers as well as by other health care providers. These providers will bill separately for their services. These providers may or may not accept the same health insurance plans as our center. The patient has the right to request from our facility a list of the other providers we use as well as their billing policies and insurances accepted.
